Announcements
- All Parent/Student Midwest Trip meeting on Tuesday, Nov. 15 at 6pm in the DSA Concert Hall. At this meeting, we will go over: itinerary, chaperone assignments, trip rules, behavior expectations, permission slips and more.
- Share our GoFundMe page on all your social media networks! Hurry…we have until the end of November to raise funds that can then be used to offset the cost of this trip.
Permission Forms for the Trip
- Durable Power of Attorney — needs to be notarized (Dorothy Hurst and Lily Casillas Garcia in the DSA Main Office are notaries and are typically available 9-10AM and 3-4PM to assist)
- Release Agreement Consent to Emergency Treatment. Note that a photocopy of the student’s health insurance card must be included with the signed release form.
- A photocopy of the student’s health insurance card.
- Acknowledgement of Risk_COVID19
- Midwest Trip Rules and Student Contract — needs to be signed by both the caregiver and the student
- Talent Consent Form — NEW FORM! VCA students ask that you give permission for your student to be part of the Midwest Clinic documentary that they are creating. If you or your student do not feel comfortable with being in the film, please let the VCA students know.
